For the Anxiety Program:
This form is to be used by General Practitioners, Pediatricians, child psychiatrist, psychologists and Ministry of Children and Family Development funded Child and Youth Mental health teams across BC, to refer a family to the Confident Parents Thriving Kids Anxiety Program
This form is to be used by Teachers, School counsellors and psychologist as well as Early Childhood Educators to refer a Student to the Confident Parents Thriving Kids Anxiety program.
This link will take you to the online referral process for all referrals to the Anxiety Program.
Note: The Anxiety Program accepts referrals from Doctors, Child and Youth Mental Health (CYMH) clinicians, Aboriginal Child and Youth Mental Health (ACYMH) clinicians, Nurse Practitioners, psychologists, teachers, school counselors and ECE’s. Referrals from other sources may be declined.
For the Behavioral Program:
This form is to be used by physicians to refer a family to the Confident Parents Thriving Kids Behaviour Program.
Please note that at this time, we are only accepting physician and nurse practitioner referrals for the Behavioral Program.
Need access to a physician?
The Access Virtual Clinic offers secure telehealth consultations with licenced B.C. Practitioners who are able to provide referrals to our services.
To get started with an Access Virtual appointment, visit https://accessvirtual.ca and register for your patient account. Once registered, you can use a computer or smartphone to start your visit.
A Care Coordinator will assist you in booking an appointment. Appointments occur within 24 hours.
